How Orthodontic Treatment Works?

Orthodontic appliances are made of metal, ceramic, or plastic. They may be removable or they may be brackets bonded to the teeth. By placing a constant, gentle force in a carefully controlled direction, braces slowly move teeth to the proper position.

Gone are the days when a metal band with a bracket was placed around each tooth. You can now choose from brackets that are clear or metallic, and in some cases, the colour of your appliance. Wires are far less noticeable, and the latest materials are designed to move teeth faster with more comfort. Today, any age is a great age to wear braces!

Duration of Treatment

Treatment time varies and can last between one and three years, depending on several factors including age, treatment method, and how minor or severe your case is. A big factor to a successful treatment is you, the patient! The more involved and diligent you are, the quicker and more efficient your treatment will be. For children, receiving interceptive, or early treatment can also help provide a quicker treatment time.

Fixed orthodontic treatments

The Controlled Arch System is one of Dr. Galella’s inventions. It might seem like other fixed orthodontic treatments but it has a huge advantage. What differs this system from other systems is its different way of approaching orthodontic principles.

This system is:

– More predictable than other systems

– Works perfectly for both adults and children and has no age limit

– Protects your jaw joint and sometimes can treat some headaches

– Works for making space in your jaw and doesn’t require pulling out healthy teeth

– It prevents gummy smile in your kid and in most cases, treat this condition in adults

– In most cases, it treats an asymmetric jaw and jaw rotation

Fixed orthodontics usually starts after all permanent teeth have grown, at the age of 12, and it works for all ages. The higher the patient’s age is, the slower the teeth would move. But once the movement has started, there are no differences between an adult’s treatment and a teenager’s.

    Traditional metal braces are the most common type of braces and are more comfortable today than ever before. Made of high-grade stainless steel, metal braces straighten your teeth using metal brackets and archwires. With metal braces, you have the option of adding colored elastics (rubber bands) for a more unique and colorful smile.

      Ceramic Clear Braces

      Ceramic braces are made of clear materials and are therefore less visible on your teeth than metal braces. For this reason, ceramic braces are used mainly on older teenagers and adult patients who have cosmetic concerns. While they are visually less prominent, they do require more attention to oral hygiene as ceramic braces are larger and are more brittle than their metal counterparts. For these reasons, ceramic braces tend to be used more on upper front teeth than on lower teeth.

      Self-ligating braces are made from the same materials as traditional braces. However, self-ligating braces do not require the use of elastics, meaning fewer appointments and less friction being placed on the tooth. Self-ligating braces come with traditional metal, ceramic, or clear brackets. They are the same size as metal braces but use a specialized clip in place of elastics to help the archwire guide teeth to place. The clip helps reduce the amount of pressure being placed on the tooth and requires fewer adjustments because there are no elastics to replace.

      In patients who have mild orthodontic problems, these clear invisible aligners come in handy. This system consists of removable aligners that have to be placed on teeth respectively – each for about two weeks, at least 20 hours each day – and they fix the teeth gradually during this period.

      Removable growth guidance appliance

      Some similar appliances have existed for years, but Dr Galella has made some important modifications in it.

      This tool is usually used for children between 6 to 9 years old. The patient has to use this for about 8 to 12 months.

      After this period, a third of children won’t need orthodontic treatments afterwards, but others need a maximum of 8 months of fixed treatment after they have reached age 12.

      This appliance is one of Dr Galella’s most significant inventions.

      This tool has to be cemented on the upper teeth for 3 to 8 months and can be used for anyone above age 9. For severe cases, we may need a longer period.

      This tool has different types, and its purpose is to widen the upper jaw in the lateral direction. Using Dr Galella’s latest system that three-dimensionally widens the upper jaw, we only need this appliance in very severe cases.

      Impacted teeth orthodontics

      Sometimes permanent teeth cannot break through the gum tissue and get locked behind the other teeth. After wisdom teeth, the most common unerupted teeth are the top canines or eyeteeth.

      In many cases, we can surgically access the unerupted tooth crown and try to bring it out orthodontically. In this process, at the time of surgery, we connect a golden chain to the crown and try to bring it out gradually.

      It is of note that many of these cases are preventable if they are diagnosed during childhood.

      TMJ Management

      Our jaw joint is a joint between the bottom jaw and the Temporal bone. The position of the lower jaw bone in the joint is highly influenced by our teeth. For example, if the top jaw is small and the bottom jaw cannot sit in its normal position, the bottom jaw will be pushed backward which can cause headaches and even many symptoms beyond the head.

      In this kind of problem, we face complications of the neuromuscular system, which is very complicated and unpredictable. If the origin of the problem is the joint, we can usually manage it, but that is not always the case.

      For the first step, we use splints, and if this step is successful, it can be followed by a successful orthodontic management. Otherwise, referral to a chiropractor or neurologist would be the next step.

      What is the best age for orthodontic treatment?

      It is usually said that children need to go under orthodontic evaluation at the age of 7, but this question does not have the same answer for every individual. Some conditions, like anterior cross bite, need to be treated regardless of age. For some conditions, it is better to wait until the age of 13. Also, some habits like mouth breathing and tongue thrust need early intervention.

      The best age for treatments that affect the jaw growth, is 7 years old until the end of the growth which is between 11 to 13 in girls, and for boys it is between 13 to 15.

      Can orthodontic treatment be successful after the age of 18?

      Nowadays, age limitations for orthodontic treatments only exist in few cases. Fixed treatments and jaw growth is possible in any age. However, the treatment duration takes longer than usual.

      What is serial tooth extraction?

      This method was done in the past and is not very popular nowadays; the least harm is that the child would lose 2 of his/her permanent teeth.

      Is tooth extraction necessary for orthodontic treatments?

      Not only this is not necessary, but also in most cases it harms the chewing system in irreversible ways. Nowadays, we can make room for other teeth by widening the jaw arch.

      Can an orthodontic treatment be a substitution for a corrective jaw surgery?

      With the latest methods in orthodontics, there is a less need for jaw surgeries. Although in some severe cases, patients may be referred to a maxillofacial surgeon.

      What is a gummy smile?

      A condition that causes the patient’s gum to be seen while smiling, about two millimeters or more. The main reason is that the upper jaw has grown downward and the upper lip is too short.

      Can an orthodontic treatment cause a gummy smile?

      In case of a mismanagement, yes, it is possible.

      Can a gummy smile be treated without surgery?

      In mild cases, a gum surgery or small treatments are sufficient. For more severe cases, orthodontics treatments are necessary, and for even worse cases, a jaw surgery is needed. The important point is that if the problem is diagnosed early, it can be prevented.

      Can an orthodontic treatment cause harm to jaw joints?

      Although any kind of treatment can affect the joints, mistreatments, especially with tooth extractions, can harm the joints, permanently or temporarily.

      One of the biggest advantages of our system is that it concentrates on joint health from the beginning.

      Can orthodontic treatments treat jaw joint disorders?

      In most cases, yes. However, considering complications in joint disorders and the impact of our neuromuscular system on joint health, not all patients respond the same to treatment.

      Can orthodontic treatments cause tooth decay or gum diseases?

      Although lack of hygiene during the treatment period can damage the teeth and gums, with proper hygiene this can be prevented.

      Can oral habits in childhood cause tooth disorders? Are these habits treatable?

      Yes. Especially mouth breathing and placing the tongue between teeth when swallowing.

      There are tools and special methods for breaking these habits.

      Can the growth of wisdom teeth cause disorders in the anterior teeth?

      No. Wisdom teeth can cause problems for other teeth, but it has been proven that they cannot affect the anterior teeth.

      An important point is that sometimes, when wisdom teeth are growing, we observe disorders in lower anterior teeth or upper ones. This is a result of jaw growth and is not related to wisdom teeth.

      Lower jaw growth continues until age 15 for women and age 22 for men.

      How long is the orthodontic treatment?

      Although this question has different answers for different patients, most of our treatments take about 1 year to 2 years.

      Can an orthodontic treatment treat bruxism and clenching?

      In most cases, yes. But because these problems are unknown and complicated, not all patients respond well to the treatments.

      What is invisible orthodontics? What are its advantages and disadvantages?

      Invisible orthodontics consists of three types of treatments:

      1. Invisible aligners like Invisalign:

      In patients who have mild orthodontic problems, Invisalign comes in handy. This system consists of some removable aligners that has to be placed on teeth respectively – each for about two weeks, at least 20 hours each day – and they fix the teeth during this period.

      2. Ceramic or transparent brackets

      The main advantage of this method is that these brackets are transparent and cannot be seen. Their disadvantage is that they are fragile and bigger than metal brackets and they are harder to brush. These brackets are usually only used for upper anterior teeth in adults. But there are no differences in functionality between these and metal brackets.

      3. Lingual brackets

      Unlike normal brackets, these brackets are placed behind the teeth.

      These brackets have 2 important weaknesses: patient’s tongue would not feel comfortable and treatment duration is longer.

      After introducing systems like Invisalign, these brackets are less popular.

      Is a small upper jaw treatable?

      Until recently, the only method for treating a small upper jaw in adults was surgery, bur Dr. Steve Galella invented a method that had a huge impact on orthodontics, and now we are able to guide the upper jaw to its natural growth without any age limit.
